Student Enrollment Requirements

 

Enrollment Packet

An Enrollment Packet is required for each student who will be attending Melissa's Private Preschool. All forms contained within the packet must be signed and submitted prior to successfully enrolling your child.

All forms must be signed and submitted with the following documentation when registering your child.

 

A copy of your child's Birth Certificate

A copy of your child's Immunization Record

Registration Fees

A non-refundable $50.00 registration fee is required for each student enrolling in the Preschool and Pre-Kindergarten programs. This fee reserves your child's space until the start of school in September and covers basic supplies throughout the school year. It is due and payable in full at the time of registration.
